Placement of Breast Implants

Dr. Lista places breast implants below or above the pectoralis muscle — the large, fan-shaped muscle that covers the chest — depending on which method will give the best result. Typically, Dr. Lista will place implants behind the muscle if the patient is thin and is experiencing little or no drooping of the breasts. Because the muscle covers the implant, there is less chance that the patient will be able to feel the implant or that the implant will create the appearance of rippling. A disadvantage of this technique is that the implant can move if there is intense movement of the pectoralis muscle.

Dr. Lista often places implants behind the breast tissue and above the pectoralis muscle if a patient has larger breasts that sag. There is a greater chance that the implant will be visible with this technique, and there is a greater risk of rippling and hardening.


The breast implant can be placed directly under the breast tissue or beneath the pectoralis muscle.

Because each of our Ontario breast enhancement patients is unique, Dr. Lista will do a thorough analysis of your body structure and medical history, and he will personally consult with you before deciding which placement option will provide the best results. Dr. Lista has extensive experience in placing both saline and cohesive gel implants above and below the pectoralis muscles. He is one of very few surgeons who perform the breast enhancement procedure using a special surgical bed so that the patient is in a sitting position. This allows Dr. Lista to accurately assess the results of the procedure to ensure that the breasts are balanced, correctly sized, and suitable for the patient's frame.

The Breast Enhancement Operation

Dr. Lista performs most breast enhancement procedures at our Plastic Surgery Clinic in Mississauga, near Toronto. All breast enhancement operations are performed by Dr. Lista. He never allows assistants or students to assist with surgery. Because Dr. Lista is chief of plastic surgery at the Trillium Health Center (The Mississauga Hospital), the procedure can be performed there, if the patient prefers. Anesthesia will be administered by a board-certified anesthesiology specialist who will stay with the patient throughout the procedure, until the patient wakes up.

The operation usually entails making an approximately one-inch incision in one of four locations to allow insertion of the implant. The incision will be made under the breast crease, around the lower edge of the areola, in the armpit, or in the belly button. In your breast enhancement consultation with Dr. Lista, he will thoroughly discuss the technique that will best suit your body and lifestyle. You have every right to make the final decision of where the incision will be placed.

The entire operation takes one to two hours. It will be necessary to have a companion to provide a ride home after surgery. Two small drainage tubes will be placed in the incisions to allow blood or fluid to drain so that swelling is kept to a minimum. These tubes will be removed the following day at our clinic.

Recovery at Home

Dr. Lista will provide the revolutionary pain-relief pump for you to use in the first 24 hours after your procedure so that you can be virtually free of pain. You'll wear this remarkable device on a comfortable belt, and it will automatically dispense a long-acting local anesthetic through very thin tubes to the surgical site. You'll be amazed at how effective it is for controlling pain. Following the pain-relief pump, Dr. Lista can prescribe medications as necessary to alleviate any pain.

After your breast enhancement procedure, you should expect to rest quietly at home for one week. Activities such as reading or watching TV are perfect for this period. If you have small children, you will need to make arrangements to have someone help you care for them during the week after the procedure. After two weeks, you will be able to begin gentle exercising. More intensive exercising can be done after a month.

Risks and Complications

The risks are few and complications are unusual with breast enhancement surgery. Dr. Lista's expertise and extensive experience help to reduce the risk of complications and ensure that the great majority of patients are extremely satisfied with their results. However, no surgical procedure is without risks. Infection and adverse reaction to anesthesia can occur. If there is excessive bleeding, a second operation may be necessary.

Capsular contracture, the squeezing of an implant by scar tissue, is the most common problem associated with breast implants. This condition may require removal or replacement of the implant. In some cases, it can be alleviated by removing or scoring the scar tissue.

Oversensitive, semi-numb, and even numb nipples have been reported by some women. Numbness can also occur in small areas near the incisions. These conditions may disappear over time, but in some cases they are permanent.

Women who have nursed a child within a year of the surgery may produce milk for a few days following the breast enhancement surgery. Dr. Lista can prescribe medication if this causes discomfort. No evidence exists that breast implants have any effect on fertility, pregnancy, or the ability to nurse.

No link has ever been found between breast implants and breast cancer. However, breast implants can affect how a mammography is performed to detect cancer. For this reason, any patient who has had breast implants should see a radiology center that is experienced in performing mammography on women with implants. In some cases, an ultrasound examination may be of help in detecting breast cancer or evaluating the implants.

Breast implants occasionally break or leak. This presents no danger to the patient. If a saline implant ruptures, the body will absorb the saltwater solution harmlessly. The gel in cohesive gel implants is solid and will not spread in the body. Because implants are guaranteed for the life of the patient, if an implant leaks or ruptures, the company that made the implant will provide a replacement at no charge.
